JOB SUMMARY:

The Clinic RN Care Coordinator provides both direct and indirect patient care within the clinic setting and off-campus locations. This role is critical to delivering premier, patient-centered care in collaboration with healthcare providers and interdisciplinary teams. With a strong sense of initiative, professionalism and accountability, the RN Care Coordinator supports continuity of care by assessing and addressing the physical, emotional, mental, spiritual and psychosocial needs of patients. This individual plays a key role in care planning, patient education and care coordination, ensuring timely communication of changes in patient status and supporting smooth transitions across the healthcare continuum.

Responsibilities Include: 

  • Providing professional nursing care to patients in accordance with physician orders, established policies and evidence-based practices
  • Collaborating with providers to assess patient needs and develop individualized care plans
  • Managing complex care coordination for patients with chronic conditions or at risk for hospital readmission
  • Monitoring patient conditions and promptly reporting changes, test results or concerns to providers
  • Facilitating referrals, follow-up appointments and communication with external healthcare entities
  • Educating patients and families on disease management, medications, procedures and preventive care
  • Supporting clinic operations and workflows to optimize patient flow and provider efficiency
  • Participating in quality improvement initiatives and population health strategies
  • Maintaining accurate and timely documentation in the electronic medical record 
  • Responding rapidly and appropriately to changes in patient condition or treatment responses

Clinic RN Care Coordinator - Job Description

 

QUALIFICATIONS:

  • Graduate of an accredited school of nursing required
  • Current Oregon Registered Nurse (RN) license or ability to obtain prior to start date required
  • Current BLS certification required or ability to obtain upon hire
  • Demonstrated knowledge and adherence to the Oregon Nurse Practice Act
  • Strong interpersonal skills with the ability to collaborate effectively with physicians, patients, clinic staff, visitors and external partners
  • Excellent verbal and written communication skills 
  • Proficient in electronic health records (EHR) systems and Microsoft Office applications, including Word, Excel and Outlook
  • Demonstrated ability to provide compassionate, patient-centered care across diverse populations
  • Ability to meet the physical and functional requirements outlined in the job description
  • High School Diploma or equivalent required
  • Must successfully pass a criminal background check, drug screen and reference checks
